HMCA (Hospital and Medical Care Association) is a specialist insurer that's been arranging cover exclusively for members of trade, professional and membership organisations since the late 1970s. It's not a household name, and there's an important reason you may not have heard of it: it doesn't advertise to the public at all.

Before anything else, here's the single most important thing to understand about HMCA: its plans generally pay a fixed benefit towards the cost of your treatment, rather than covering the full cost the way traditional comprehensive private medical insurance from insurers like Bupa or Aviva does. For routine treatment this rarely matters. For something serious and expensive, like major cancer or heart treatment, it can mean a real, sometimes substantial, gap between what you're covered for and what the treatment actually costs.

This is genuinely the detail that matters most if you're comparing HMCA against a traditional comprehensive insurer, so we're not going to bury it.

What "benefits towards treatment" actually means

HMCA's own website describes its entry-level plan as offering benefits "towards" in-patient treatment, rather than covering it in full. This is a meaningful structural difference from insurers like Bupa, Aviva or AXA, who typically pay the full cost of eligible private treatment (subject to your chosen hospital list and any excess), regardless of how expensive it turns out to be.

With a fixed-benefit model, you're paid a set amount toward a given procedure or condition category. For routine, lower-cost treatment, this difference may barely register. For major, expensive treatment, particularly complex cancer care or heart procedures, the gap between what's paid and what the treatment actually costs can be substantial, and you'd be responsible for covering the rest yourself.

A real example, from a recent customer review

One recent, detailed Trustpilot review described a family member needing urgent heart surgery, with the private treatment estimated at around £64,000. The amount HMCA's cover paid toward it was reported as roughly £16,000, leaving a substantial shortfall the family would have needed to cover themselves. We can't say this reflects every claim, but it's a real, recent, credible illustration of exactly the risk described above.

None of this means HMCA is a bad product, for many members, especially for routine treatment or as a lower-cost alternative to going without cover entirely, it can genuinely work well. But if you're weighing it against a traditional comprehensive policy, it's important to compare like with like, and to ask HMCA directly for their current benefit schedule for any condition you're particularly concerned about before you commit.

Can You Actually Buy HMCA Cover?

Only if you belong to a qualifying trade, professional or membership organisation. HMCA works with several hundred groups, ranging from professional bodies and trade unions to hobbyist clubs, alumni associations and consumer publications, so the list is broader than it might first sound.

If you're not already a member of a qualifying group, HMCA generally isn't available to you directly. It's worth checking whether any membership, trade body, union or subscription you already hold happens to qualify before assuming this option isn't open to you.

NHS Cash Benefit

Choose NHS treatment instead of going private, or need an emergency admission, and HMCA pays £100 a night for up to 30 nights, tax-free cash you can use however you like.

A Real Path For Pre-Existing Conditions

Switching Without Losing History

If you transfer from another insurer, HMCA has stated that claims for acute conditions that originated under your previous plan will still be honoured, rather than starting your underwriting from scratch.

No Medical Exam, No Age Limit

HMCA offers guaranteed acceptance with no medical examination required and no upper age limit, a real point of access for people who might struggle to get accepted elsewhere.

A few smaller extras worth knowing about too: sports injury treatment and a personal accident benefit are included on the core plans, and HMCA can pay your specialist directly rather than requiring you to pay upfront and claim the money back.

Where HMCA Sits On Independent Ratings

No Defaqto or Which? rating

Unlike Bupa, Aviva, AXA and the other major insurers we review, HMCA doesn't currently appear on Defaqto's or Which?'s mainstream rating panels, most likely because it's a niche, membership-only insurer outside their typical review scope, rather than any reflection of quality one way or the other.

Its Trustpilot presence is smaller and more mixed than the larger insurers too. It's worth reading a handful of recent reviews yourself, including the less positive ones, before deciding, rather than relying on a single average score.
