Saga is the only mainstream UK health insurer built specifically for the over-50s, and its underwriting arrangement changed in a way many reviews still haven't caught up with.

Saga is underwritten by Bupa Insurance Limited, not AXA PPP as older reviews state. Bupa took over on 1 December 2024, so the claims-paying insurer, hospital network and clinical authorisation behind every Saga plan are Bupa's, wrapped in Saga's own plan structure, pricing and branding.

Who Actually Underwrites Saga Health Insurance?

Worth getting right, because a surprising number of reviews still have it wrong.

Bupa Insurance Limited, not AXA PPP

Saga's private medical insurance was historically underwritten by AXA PPP Healthcare. Saga appointed Bupa to replace them, and the switch took effect on 1 December 2024. Every Saga health plan written since then sits on a Bupa contract.

In practical terms, Bupa is the regulated insurer legally responsible for paying your claims, and you get Bupa's recognised hospital network and clinical authorisation framework. Saga sets the plan design, the four pricing tiers, the rates and the customer experience.

It isn't a rebadged Bupa policy though. Saga runs four tiers against Bupa's two individual plans, prices them differently, and offers features Bupa's own direct product doesn't, including the optional hypertension cover below.

What this means when you compare

If you're weighing Saga against Bupa, you're not choosing between two different insurers. You're choosing between two ways of buying cover from the same one. Agree to use the NHS where treatment is available within four weeks and cut your premium. Most insurers set this bar at six weeks.

The self-referral list goes further than most

Beyond cancer, musculoskeletal and mental health, Saga also allows direct self-referral for changes in your breasts, moles or skin lesions, blood in your urine, coughing up blood, and an elevated PSA result. For an over-50s customer base, that's a well-chosen set of symptoms to remove the GP bottleneck from.

Support and Saver Plus hold notably weaker Defaqto ratings than Secure and Super. If Saga's 5-star rating is what drew you in, that rating belongs to Super alone. Confirm the tier before comparing the price against anything else.

Saga's No-Claims Discount Punishes Small Claims

Saga's no-claims discount is refreshingly simple to understand, and that simplicity is exactly the problem.

Any claim, any size

A claim of any value automatically drops your no-claims discount by two levels. There's no small-claims threshold and no sliding scale based on the amount.

The drop from a £40 claim is the same as the drop from a £4,000 claim.

Compare that against Bupa, which operates a small-claims allowance, or The Exeter, where claims under £300 don't touch your discount at all. On Saga, a single minor claim carries the same renewal penalty as major surgery.

And it applies policy-wide

The no-claims discount sits at policy level rather than per person, so if you've added a spouse or children, one person's claim affects everybody's discount. Combined with the two-level drop, that makes Saga meaningfully better suited to individual buyers than to couples or families.

How Much Does Saga Cost?

Two data points give a useful picture. Which?'s 2025 testing found Saga was the cheapest of six leading insurers in an over-50s comparison. Separately, Saga tends to run around 5 to 15% cheaper than equivalent Bupa cover for the over-50s, with the gap widening at older ages and higher tiers.

Published ranges put Saga premiums broadly between £75 and £200 a month depending on age, postcode, tier, excess and options. Given the same underwriter sits behind both, that price gap against Bupa is the clearest reason to consider Saga.

Does Saga cover pre-existing high blood pressure?

It can, which is unusual. Saga offers an optional hypertension cover for an additional premium where the condition has been controlled for two years or more, subject to underwriting. Bupa's own direct product doesn't offer this, so it's one of the features that makes Saga more than a rebadged Bupa policy.

Is Saga good for a couple or family?

Less so. You can add a spouse or children, but there's no couples or family discount, no parental accommodation benefit, and the no-claims discount applies policy-wide, so one person's claim penalises everyone. Saga suits individual over-50s buyers considerably better than households.
